Biometric and Anterior Segment Parameters Changes After Uneventful Ahmed Glaucoma Valve Implantation

Abstract:

Purpose: To investigate changes in anterior chamber parameters as well as axial length, keratometry and refraction after uneventful Ahmed Glaucoma Valve implantation.Methods: In a prospective study patients with refractory galucoma who were condidate for Ahmed valve surgery were included in this study. Patients with history of any corneal surgery were excluded. Refractive status, intraocular pressure, axial length, anterior chamber parameters including corneal anterior and posterior mean keratometry, K flat, K steep, central cornea thicknesses, and anterior chamber depth, volume and angle were evaluated at baseline and one and three months after surgery. Results: 20 patients were included. Mean IOP at baseline was 33.4±12.3mmHg that significantly decreased to 14.6±6.2 mmHg at one month (P<0.001) and 13.5± 4.3mmHg at 3 months after AGV implantation. (P<0.001) Mean number of medications was 3.6±1.3 at baseline which significantly decreased to 0.8±0.8 at one month (P<0.001) and 1.0±1.3 at 3 months after AGV implantation. (P<0.001) Axial length decreased significantly from 23.69± 1.95mm to 23.47±1.91mm (P<0.001) at third month. Mean spherical equivalent was 0.62±2.72 at baseline, which changed to 0.52± 3.03 at 3 months. (P<0.813) Anterior meanK changed from 44.20±2.21 to 44.45± 2.39 at one month (P <0.05) and 43.61±1.95 at 3 months. (P<0.81) There was no significant chage in central corneal thickness, anterior chamber volume and posterior keratometry at 3 months after Ahmed glaucoma valve implantation. Conclusion: Ahmed glaucoma valve implantation had a significant effect on axial length at 3 months after surgry but its effect on keratometry and other anterior chamber parameters was not significant
