A Case report of 0a Pyoderma Gangrenosum (P.G.) caused by ulcerative colitis and healed by advanced methods of wound care

Abstract:

INTRODUCTION:P.G. is a neutrophilic dermatosis as a part of a complex systemic auto inflammatory process. The statistics indicate an incidence of 0.3-1.0 of per 100,000 inhabitants. Accordingly, P.G. is considered an orphan disease and so far, the exact etiology of P.G. remains unknown. The presence of abnormal neutrophils and T-cells lead to immune dysregulation with increased levels of inflammatory mediators. Patients with Pyoderma Gangrenosum have atypical, painful, deep and exudative ulcers. The healing process for these wounds takes long and leads to an impaired quality of life. Due to excessive pain and exudates, the treatment is complex and difficult.AIM-CASE REPORT: The aim of this study is to report a case for Pyoderma Gangrenosum (P.G.) caused by ulcerative colitis, healed by advanced methods of wound care.CASE REPORT: The case was a 30-year-old man with severe (P.G.) ulcers found in his axillary region. The patient received treatment for 18 days, using traditional dressing and Tacrolimus ointment but no signs of healing were eventually found. Alternatively, the wounds were washed with standard saline solution and Silver alginate was used to control exudate and prevent secondary infections. Medical honey and hydrogels were used for autolytic debridement and afterward, the wound was closed using superabsorbent secondary dressings. By using this method, the wounds were healed within 4 weeks. We choose this case for two reasons: 1) The placement of the wound in axillary region, as these wounds are typically found in lower limbs. 2) Most of patients in Iran rarely receive treatments using modern dressing techniques.CONCLUSION: According to the results of this study, advanced dressings have better efficiency in treatment of Pyoderma Gangrenosum wounds compared to use of traditional treatments.
