Preventing foot drop in intensive care units

Abstract:

Introduction: Patients admitted to the intensive care units often suffer from various complications due to movement limitations. One of the most severe problems of immobility is a plantar flexion contracture (commonly known as foot drop), due to shortening of the Achilles tendon and common peroneal nerve (CPN) dysfunction results in paralysis of ankle dorsiflexion muscles , as the foot is allowed to fall and remain in plantar flexion the resultant decrease in range of motion (ROM) of the ankle impairs gait, increasing the risk of falling. Case Presentation: We report a 65 years old female admitted in ICU due to CVA and GIB .she had no history of any illnesses and neuromuscular disorders. after 20 days of hospitalization she had left sided -foot drop due to immobility, inactivity and not receiving nursing cares needed for prevention of foot drop. Discussion: nurses can prevent this complication in intensive care units by doing simple cares such as: using simple braces and tools that are available to prevent foot drop. If these tools are not available you can keeping the foot in a natural position to prevent foot drop (planter flexion), create a backrest for the foots by using the patient s bed as a support surface for the foots with 90 degree. Checking the patient nerve damage daily , especially the peroneal nerve because of increased risk of foot drop. Also exercises for ankle range of motion (ROM) should be conducted twice daily. therefore you as a nurse can prevent foot drop by give cares such as these cares we mentioned.
