Abstract:

PURPOSE: Management of colorectal deep infiltrating endometriosis (DIE) remains a dilemma tothe gynecologic surgeons. Several laparoscopic approaches including the rectal shaving, discresection and segmental resection are available for management of these lesions. The aim of thecurrent study is to report the outcome of laparoscopic management of patients with colorectal DIEin our center.Methods: This cross-sectional study was conducted during a QA5 period in our center. Weretrospectively include all the patients with confirmed diagnosis of the gastrointestinal DIE durind 5years interval(2080 patients). All the medical charts were reviewed and the demographicinformation, the preoperative diagnostic evaluations, surgical approach, intraoperative, andpostoperative findings were recorded. A 6&12 month interview was conducted to evaluate thefunctional outcome.Results: Among 520 patients (with mean age of 34.2 ± 5.9 years) with gastrointestinal DIE. Weperformed laparoscopic rectal shaving or peeling in 205 (39.4%) patients with colorectalendometriosis while 226 (43.4%) patients underwent segmental resection and anastomosis, and 64(12.3%) were treated with disc excision and 25 (4.8%) appendectomy. Ileostomy was performed in2(0.38%) patients ,Peritonitis was recorded in 1(0.19%) patient. four (0.76%) rectovaginal fistulasand 1(0.19%) bladder atonia .Conclusion: Laparoscopic resection of the colorectal DIE is a feasible and safe method beingassociated with low complication rate and favorable functional outcome by expert surgeon.
