Evaluation of Epidemiological considerations, among Iranian women diagnosed with Gestational trophoblastic neoplasia: A Retrospective 10 years follow up

Abstract:

Background and Aim : The aim of this study was the evaluation of epidemiologic, clinical characteristicand outcome after treatment in patients with Gestational trophoblastic neoplasia (GTN) at Imam Hosseinhospital (IHH).Methods : we retrospectively reviewed medical records of women diagnosed with GTN from January 2006to December 2017. Then, important factors including disease diagnosis, treatment, outcomes and followupdata were analyzed by SPSS software.Results : during 10 years 222 women with GTD were recorded, among them 30 patients diagnosed withGTN (13.51%). Finally, 22 patients with GTN had complete records in the hospital. The mean age and themean level of pretreatment β human chorionic gonadotropin (β hCG) of GTN patients were 36.04 ± 9.2year and 79322.1 mIU/ml, respectively. 73 % of patients had the interval time from antecedent pregnancyto GTN development less than 4 months and also 59.1% of GTN patients had A+ blood group. 81.8 percentof the patients had choriocarcinoma and 18.2% had invasive mole. Moreover, 45.5 % of the GTN patientswere high risk group (score≥7) and also 40.9 % patients had metastatic diseases. There was theca-luteincyst in 41% of the patients, which had a significant (P<0.05) association with the disease risk of GTN. Inaddition, 22.7 % of patients responded to single drug chemotherapy with methotrexate, 27.3 % treated withmulti drug chemotherapy with EMACO. Finally, our findings showed that both of outcomes of survivaland pregnancy rate were 100% in the GTN patients after treatment.Conclusion : Choriocarcinoma was higher type of GTN in this referral hospital. If GTN recognized ontime and treated suitably, this disease goodly controlled and cured. For better evaluation of patients withGTN and the outcome of different type of treatments larger sample size studies based on organized datacollection and better registration is suggested.
