Comparison of light microscopy and Nested polymerase chain reaction methods in diagnosis of malaria blood smears in south-eastern of Iran

Abstract:

Introduction and Objective: Malaria is still one of the most life threatening infection. According to the WHO reports there were 215 million cases of malaria in 2015. The aim of this study was comparison of light microscopy (LM) and nested polymerase chain reaction (nested PCR) methods in diagnosis of malaria blood smears in south-eastern of Iran.Materials and Methods: In total, 95 blood smears from Kerman, Sistan Baluchestan and Hormozgan provinces were used in this study. We evaluated the smears by LM to initial diagnose the parasite. Then, DNA from each smear was extracted by boiling and Gene All kit. For nested PCR we used the genus and species primers. Finally, the results of the LM were compared to the nested PCR results.Results: In total, 95 of 95 (100%) cases were positive by LM while, 92 of 95 (96.8%) of cases were detected as malaria by nested PCR. From 92 positive cases 4 (4.34%), 36 (39.13%) and 54 (58.69%) belong to Kerman, Hormozgan and Sistan Baluchestan, respectively. Nested PCR result confirmed LM result except 7 samples. These samples involve 2 Plasmodium vivax and 5 Plasmodium falciparum in LM method while all of these samples were detected as mix infection of P. vivax and P. falciparum with Nested PCR.Conclusion: Our finding highlights an important concern in the diagnosis of malaria by LM method which can lead us to false results and also is less reliable compared to molecular methods.
