Introduction: Overweight and obesity in childhood is becoming a global epidemic which may result in increased morbidity and mortality during young adulthood. The increasing incidence of obesity globally may be a consequence of lifestyle behaviors and environmental factors. Therefore, the purpose of this paper is to explore the prevalence and multidimensional factors related to childhood obesity and overweight in northwest Iran. Methods: This cross-sectional study was carried out in two phases. In first phase, weight and height of 4,400 children aged 6-12 y was measured. Body mass index (BMI) was calculated and based on the US Center for Disease Control and Prevention (CDC) criteria, overweight and obese children were defined. In second phase, 54 children with overweight or obesity and 115 normal children were selected to answer questionnaires regarding daily physical activity and sleeping hours, food intake, birth weight, as well as parental education, vocation, weight and height. Two models were used to examine factors associated with overweight or obesity. Statistical analysis was performed using STATA version 12 software (StataCorp, College Station, TX, USA).Results: The overall prevalence of children who were overweight or obesity was 15.4%. Mother obesity (OR = 2.5, p = 0.01) and obesity history in family (OR = 1.7, p = 0.01) was significantly associated with higher risk of overweight and obesity. We conducted two models to discover best model for predicting overweight or obesity in childhood periods. In model one that composed of sex, age, birth weight, breast feeding duration, screen time, sleep duration, father and mother jobs, father and mother educations, physical activity severity, house prosperity, BMI of mother and father, physical activity severity was found to be a protective factor against being overweight or obese (OR = 0.85, p = 0.02) and BMI of mother (OR = 1.24, p = 0.02) and father (OR = 1.42, p = 0.03) were associated with an increased risk of being overweight or obese. In model two, intakeof energy, protein and fat was added to the model. There were not any significant differences between two models (p = 0.36) and the predicting power of the first model was higher than second model. Conclusion: Our findings show that parenteral obesity and physical activity are associated with childhood obesity and overweight. Although the genetic cause for obesity is not to be ignored, lifestyle behaviors of parents are more likely to be closely related to the association between childhood overweight and parents obesity. Therefore, developing strategies that consider the parents education on lifestyle changes to reduce childhood overweight and obesity would be the most effective way to prevent and manage this multifactorial health concern.
