Abstract:

Background and aim: Chronic obstructive pulmonary disease COPD is a chronic disease characterized by progressive airway obstruction that affects many people in the world. It is the fourth cause of death among people in the world. Also, people with COPD generally have a depressed mood, a sense of frustration, and lack any effort to improve their well-being; and also They are unaware especially those living in remote areas about self-management and managing their illness. COPD imposes heavy costs on the global health system, which is a large part of these costs related to admission to the hospital. Continuous interventions also mean that caring for patients outside the hospital will continue, on the other hand, self-care is also the transition of caring for health care workers to individual care.Materials and Methods: In this paper, articles from 2001 to 2018 in English with keywords such as self-management support, chronic obstructive pulmonary disease, continuity of care, nursing care, plus articles in the Persian language from 1381 to 1397 with keywords Quality of life, chronic obstructive pulmonary disease, home care, hospital admission was chosen. All articles are selected from Pubmed, google scholar, and SID databases. result: One of the important tasks of nurses is to improve the knowledge and quality of life of patients with post-hospital care and education. five studies show that continuous and focused nursing interventions at home can improve the quality of life of COPD patients. Of course, Highlighting a problem in the field of study is the role of a nurse as an indicator has not been evaluated. Most research studies show that education is not only effective but if this training is interventional, such as daily phone calls, it can be very effective and patients and their families will welcome the plan.Conclusion: According to research, continuous nursing interventions CNI can reduce patient hospital readmissions, length of hospitalization and hospital costs compared to control groups.
