Improving delivery and paying attention to maternal mental health during labor and birth satisfaction in prime parous women

Abstract:

Objective: Labor pain is one of the most severe pain that most women experience their lives and affects the health of mother and infant, emotional relationship between them and desire to have a child in the future. This study had been done for improving delivery and paying attention to maternal mental health during labor and birth satisfaction in prime parous women.Methods: In this randomized clinical trial, 90 prime parous women candidate vaginal delivery and referring to Sina hospital maternity affiliated to Kordistan University of Medical Sciences were randomly assigned to test and control groups. Before intervention, demographic-obstetric questionnaire and Spielberger anxiety inventory completed by two group and pain score determined with numerical rating scale. Then in active phase, women of the test group received an ice pack every 30 minutes for 10 minutes on their sacrum and control group received rooming labor routine care. Pain severity assessed every 1 hour and the levels of anxity at dilatations of 4-8 cm and 8-10 cm, too. Also birth satisfaction measured 24 hour after normal birth with birth satisfaction scale- revised. Data was collection by SPSS software and analyzed with the Independent T-test, Chi-square, Fisher exact test, repeated measure and Bonferroni test.Results: The two groups haven t statistical differences in demographic-obstetric characteristics and pain intensity and anxiety before the intervention. after intervention, following labor progress, pain intensity increased in two group but increasing pain was in control group with significant difference more than test group (P<0/001). Also compared with control group that level of anxiety increased, in test group level of anxiety reduced with significant difference (P<0/001). In test group, according to Repeated measure, there was significant difference between level of anxiety before and after intervention (P<0/001). Although level of satisfaction was in test group more than control group, but according to T-test, there wasn t significant difference between two group (P=0/24).Conclusion: The results showed that application ice pack every 30 minutes for 10 minutes in active phase, reduce pain and anxiety. So cryotherapy recommend to midwives and mothers as simple, available, inexpensive and noninvasive technique. It will increase the satisfaction and pleasure of delivery.Research message: Health policy makers and managers to make the labor and delivery process more enjoyable and pleasure and reducing the costs and complications of a variety of medication methods can relieve the pain of Liber and delivery by applying the ice bag alone or in combination with other complementary medicine in midwifery wards.
