Case report: Treatment of diabetic foot ulcer by using team work technique and modern dressings

Abstract:

The purpose: The most common complication of diabetic is ulcer in lower extremity. It s due to ischemia, neuropathy or neuro ischemic. DFU treatment consists of 4 principles of offloading, glucose control and diet, infection control and improvement of blood supply in foot. The purpose of this study was to determine the effect of team work on speeding up the healing process of diabetic wounds. We report a DFU Case Treatment by using team work technique and modern dressings- The main materials and methods used: Case report: data collection of patient history, family, biochemistry tests. Collaborative Therapy and Teamwork: Step One: Refer the patient to a vascular specialist, endocrinologist, nutritionist, and infectious specialist. Step 2: Patient education. Step 3: Debridement of necrotic tissue and use of alginate and collagen dressings. In this case report, we assess a 57-year-old man with past history of DM, HTN, old CVA and hemiplegia. Within a week, a pustule is developed on the big toe and became a large blister within 5-6days. Three days later, the blister is ruptured and the necrotic wound developed. Patient goes to hospital with main complaint of wound infection. In his laboratory tests is reported: ESR: 158, BS: 263, BUN: 27, Cr: 1.4, WBC: 7600, Hb: 13.6, INR: 1.4. Wound Specifications Included: DFU: G3, size: 3x5.2. Necrotic tissue: 65%. Exudate: nil. Odor: nil. Surrounding Skin: dry. Wound Edge: Pink.- The main results obtained: In this case, teamwork with the presence of a wound specialist, dedicated specialists and patient s cooperation and his educable resulted in minimum treatment duration (17) days and maximum satisfaction (100%). This result indicating the importance of our team work participation in patient s ulcers improvement.- The main conclusions of work: Diabetic patients are at greater risk of developing foot ulcers than other people. DFU s complications can be prevented by properly educating diabetic patients such as preventing of ulcers or refer to specialists at early stage of wounds. This strategy cause to reduce the duration of treatment, hospitalization, and costs.
