Background and Aim : overall incidence of sex cord-stromal neoplasms was 0.2 per 100,000 womenbased on the United states cancer database from 1992 to 1999. In 57 percent of the women, disease wasconfined to the ovary. We present the case due to rarity and challenges during diagnosis of disease.Methods : Case Presentation: Married 54-years-old post-menopausal woman presented at the clinic withradiologic findings in favour of the adnexal mass near the Right ovary without any specific symptom.Magnetic resonance imaging disclosed a 60 x 34 mm solid lobulated homogenic hypoechoic mass in theright adnexa, which extended to Para rectal fossa just adjucent or in the right ovary. There was not any fluidfound in the pelvic and the endometrial thickness was normal.Tumor markers include CEA, CA125, CA19-9, inhibin, alpha-fetoprotein, and other routine lab results were normal, whereas HE4 was elevatedResults : . Due to the disposition and elevated HE4, patient was taken to the operation room with suspicionof malignancy. Fibroid like mass measuring roughly 60 mm, was removed from the right Broad ligament.Pathological investigation during surgery was suspicious of benign stromal lesion (fibroma), but becauseof the rarity and HE4, TAH and BSO were conducted. . patient was discharged from the hospital withoutany complication and routine follow-ups were requested. Furthermore, pathologic findings showed ovarianstromal tumor in favour of fibroma.Conclusion : We encountered 54-years-old post-menopausal woman with sex cord-stromal tumor ofoblique ligament with elevated HE4 who underwent TAH and BSO in addition to tumor Resection becauseof displacing of the Lesion and suspicion for malignancy.
