Abstract:

Introduction: Considering that today's dramatic advances in public health have been made, the number of patients with pulmonary tuberculosis in recent years is significantly increasing. The purpose of this study was to determine the difference and comparison of the results of PCR, culture and smear in different samples of patients referring to the TB Reference Laboratory of the Masih Daneshvari Hospital. Materials and Methods: In this descriptive study, all samples for which PCR, culture and smear were performed from ۲۰۱۲ to ۲۰۲۲ were examined. In suspicious cases, PCR-RFLP was performed for the samples and the answer of this test was considered as the final answer. Finally, PCR responses were compared with smear responses and direct culture of samples.Results: Out of ۶۴۲۲ samples, in ۵ samples, all three tests were positive for smear, culture and PCR. In ۱۳ cases, smear and culture were positive and PCR was negative. In ۳ samples, smear was negative and the other two tests were positive, and in ۴ samples, culture was negative and the other two tests were positive. In ۳ cases, only culture was positive. In ۴ samples only smear and in ۲۲ samples only PCR were positive. The PCR method had a sensitivity of ۳۵%, specificity of ۹۸%, positive predictive value of ۳۵% and negative predictive value of ۹۸% compared to the two methods of smear and culture.Conclusion: Due to the high specificity and low sensitivity of PCR method in different samples of patients with pulmonary tuberculosis, this method can be very useful for accurate, rapid and timely diagnosis of patients with Mycobacterium tuberculosis and patients with non-TB mycobacteria. Thismethod can also be cited along with the two methods of smear and culture as determining the health of individuals by considering the high specificity of the test.
