Abstract:

This is a retrospective study on clinical course and treatment response of patients with thyroid hot nodule, referred to nuclear medicine department of Shariati Hospital from ۱۳۶۰ to ۱۳۷۷. From the total ۵۸ patients with average age of ۴۳ yr and incidence peak of ۴۰ yr, ۸۱% were female and ۱۹% were male. ۶۴% of the patients were thyrotoxic and ۳۶% were euthyroid at presentation. Thyrotoxic symptoms and signs increased with age (P=۰.۰۴). ۵۷% of patients received I-۱۳۱ therapy without premedication. ۷% received antithyroid drugs first and then underwent iodine therapy. From ۳۶% of the patients who were euthyroid at presentation, ۱۵.۵% became thyrotoxic on follow up studies and received I-۱۳۱ therapy, but ۲۰.۷% remained euthyroid without receiving any therapy. The average of first and cumulative radioiodine doses were ۱۶.۹ mci and ۲۱.۸ mci, respectively. Treatment response during ۶ and ۱۲ month follow up were ۸۰% and ۸۶.۷%, respectively. The more administered the first dose, the better was the response (P=۰.۰۳). Men appeared to need more I-۱۳۱ dosage and times of therapy as compared to women. Post radioiodine hypothyroidism incidence was ۷.۵% during one yr follow up. No meaningful relation was found between times of radioiodine therapy and thyroid or nodule size.
