The risks in the otolaryngology have a wide range of mistakes most of which are preventable. Therefore the present study was conducted with the objective of the risk assessment of the processes of Ghaem otolaryngology surgery department by the health failure mode and effects analysis (HFMEA) 2013-2014. In this research with a mixed method (qualitative and descriptive cross sectional qualitative), the modes and effects or HEMEA method were defined and analyzed. In order to trace back the reasons of the risks, the Eindhoven classification model (ECM) and for offering remedial recommendations, the theory of inventive problem solving (TRIZ) were implemented. During the 5 selected procedures by voting method using rating, 22 processes and 48 sub-processes, 218 failure modes with the HEMEA were recognized, 8 risk modes (%3.6) were found as non-acceptable risks and were transferred to the decision tree. The main root cause for (hazard score >=4) were: (14.34%) technical- related factors; (31.9%) organizational- related factors; (45.3%) human- related factors and (7.6%) other factors. Creating and revising the guidelines as well as a clear executive methodology, the involvement of the patient during the medical procedure, re-engineering and supervising the procedures, training on the recommendations and guidelines, improvement of the connection among the sectors, were included in the executive strategies for the improvement and enhancement of the otolaryngology surgery.
