Abstract:
Medical errors are considered as one of the majorchallenges of the health throughout the world system andimpose many costs on society every year. The losses thatcaused by mismanagement of these hazards are not only forthe patient, these errors can affect the loss of goodphysicians and hospitals reputation. Threats of Medicalerrors in the healthcare system, predispose the movementsand efforts to minimize the statistics of these errors,mortality and disability. The aim of this study is to providea systemic model to analyze the dynamics of the healthcaresystem risks to eliminate or reduce the damages of services.In this paper, we could address to identify factorsinfluencing the adverse events due to medical errors withusing system dynamics (SD) approach and shows theirimpact on our healthcare system and clinical errors.According to the proposed model, creating a safety culturein the health sector, using indicators of overall perceptionof safety, non-punitive response to error, organizationallearning, responsibility, supervision and teamcommunications play an important role in reducing adverseevents is due to medical errors, in addition to increasingknowledge and skills of staff through training on the jobcan also have a significant impact in reducing theseincidents. Using a systemic approach in dealing with theproblem of medical errors led to support of physicians andwill result encouraging themselves to express the errors.Expressed the errors will lead to the system detection andwill help the error analysis and preventive measures.
