Helicobacter pylori (H. pylori) is a highly successful pathogen of the human stomach, which is exceptionally colonized to be stable for decades; and plays an important role in the development of gastric cancer (GC). H. pylori CagA is the first bacterial oncoprotein to be identified in relation to GC. In this study, we have investigated new clonal lineages of the H. pylori cagA gene in Iran and their associations with GC. We identified a new clonal lineage of cagA gene in Iranian H. pylori strains (denoted cagA lineage II) by using a Neighbor-Joining tree clustering framework, which was the intermediate between the two Eastern and Western dynasties; and examined its association with GC, in 191 H. pylori isolates recovered from patients with digestive disorders by PCR amplification. Data were collected and analyzed using SPSS version 19. The subjects included 128/191 with non-atrophic gastritis (NAG) and 63/191 with GC. Of the 191 patients, 109 were males and 82 were females. In general, the cagA+ genotype could be determined in 105 (55.0%) strains. Of them, 26 (24.8%) had cagA lineage II genotype (NAG: 19.0%, and GC: 42.3%) and results of simple logistic regression analysis showed a strong correlation between this genotype and the risk of GC [OR (95% CI) = 3.129 (1.198-8.171); P = 0.020]. It is proposed that the cagA lineage II genotype may be considered as a useful predictive tool for assessing the risk of gastric cancer in Iran
