Introduction: Breast cancer is a heterogeneous disease with different subtypes that are predictive of patient prognosis. Despite development of surgical methods and oncology treatments, the prognosis of breast cancer is still poor. Identifying new biomarkers can be useful in prediction and developing novel therapeutic targets. The aim of this study was investigating the prognostic role of FOXA1, a pioneer transcription factor,in breast cancer.Methods: In this review, we use PubMed, google scholar and science direct databases by focus on the association between FOXA1 expression and breast tumor prognosis. The key words used are as follows: breast cancer, FOXA1 expression, luminal subtype and basal subtype. The language of this systematic search was English Results: FOXA1 expression positively associated with luminal subtypes that are well-differentiated and good- prognosis tumors and negatively associated with basal-like breast tumors that are poor- prognosis subset. It seems that FOXA1 acts as a tumor promoter in initial stages, but as a tumor suppressor in later stages of breast cancer. Recently it has been shown that FOXA1 directly represses a subset of basal genes,while maintains luminal genes expression. FOXA1 silencing results in migration and invasion of luminal cancer cells that are characteristics of basal subtype cells.Conclusion: Giving to FOXA1 correlates with favorable prognosis and controls plasticity between luminal and basal breast cancer cells, advancement of new pharmacological agents to elevating FOXA1 expression in basal like breast cancer can be useful for making luminal differentiation and using hormone responsiveness
