Symptomaticpatients usually present with cough and dyspnea. In this paper, we report a 38 years old man whosuffered from chest pain for 3 month. CT scan findings revealed scattered nodules that were lessthan 1cm.Spirometry was normal and the arterial oxygen saturation at room air was 85%. Openlung biopsy revealed DIPNECH. Most patients with DIPNECH have been elderly women, whopresent with cough and dyspnea, but we report a young man with presentation of chest pain andhypoxia without dyspnea. DIPNECH can occur in male and female in any age.
