Kissing Vessels and Neutrophilic Cryptitis Are Hint of Presence of H.pylori Induced Gastritis

Abstract:
BACKGROUND: Infection of the gastric mucosa by helicobacter pylori is primarily responsible for gastritis, gastric ulcer, adenocarcinoma, and lymphoproliferative disorders. The aim of the study was to determine the prevalence of association between H pylori volume with congested vessels, cryptitis, lymphoid follicles ,intestinal metaplasia and type of gastritis in H&E and IHC staining. METHODS: 60 patients with gastritis were the subjects of the study. Biopsy materials were taken from two sites including the antrum and corpus. The endoscopic and histopathological findings were documented. H pylori volume (mild,moderate,severe)determind in IHC staining with monoclonal antibody. The prevalence of congested vessels, cryptitis, lymphoid follicles ,intestinal metaplasia according to the type of gastritis and their relation with H. pylori density were determined. RESULTS: Type of gastritis was ;Chronic gastritis (43%),Chronic active gastritis (43%) and Chronic congestive gastritis(14%). H. pylori was detected in 50 (84%)patients in IHC staining. Congestion was seen in 98% of patient in upper lamina properia, in 37% upper and mid lamina properia and 3.3% in upper and deep lamina properia. H. pylori density was seen in the glandular lumen attached to the congested vessels .Cryptitis was seen in 60% of cases, that 50% with mild Hpylori, 34% moderate and 16% with severe H pylori colonization. lymphoid follicles was seen in 66% cases. There is positive correlation between H pylori volume and kissing vessles (p value <0.05) and cryptitis .No significant relation was established between intestinal metaplasia, lymphoid lesions and H. pylori density. CONCLUSION: congested vessels and cryptitis in gastric mucosa involving both antrum and corpus significantly correlated with H. pylori infection, H. pylori density and type of gastritis in adult.
