Evaluation of the prognostic value of perineural invasion in patients with gastric cancer who underwent curative resection

Abstract:

In this study, we investigated the relationship between PNI (examined with H&E staining) & other clinicopathologic factors and analyzed the effect of PNI on overall survival in patients with gastric carcinoma who underwent curative resection. Recently, Bilici et al. indicated that the incidence of PNI in gastric cancer was higher, and it was related to the progression of patients with gastric cancer. The study of Duraker indicated that the positivity of PNI is 59.6 % and with the progression of gastric cancer, the incidence of PNI increased. Materials & methods 142 patients with gastric carcinoma had undergone curative gastrectomy at Shohada & Taleghani Hospitals, during Jan 2006 and Dec 2013.Among the potential participants, 46 patients out of 142 were excluded from this study, due to lack of data follow up .Ultimately, 96 patients were included for the analysis. Clinical information about age, gender, resection type, tumor location, histological type, tumor stage, tumor size, histologic grade, lymph node involvement, the depth of tumor invasion, lymphatic/ blood vessel invasion obtained from patients routine postoperative pathological results. Variables compared with chi-square & Fisher Exact test as appropriate to compare the distribution of individual variables between groups. Survival rates were estimated by the Kaplan-Meier method. We evaluated the effect of perineural invasion as a prognostic factor on survival by Cox regression analyses. All analyses were performed using SPSS 18.0 (SPSS Inc., Chicago, IL, USA) software. The statistical level of significance was defined as p < 0.05.results Perineural invasion was more frequent in patients who was > 65 year old, male , diffuse type gastric adenocarcinoma, poorly differentiated type, located in two third or more and lower one-third part of stomach, tumor larger than 5 cm and greater depth of invasion and more lymph node metastasis and greater tumor stage ,more lymphovascular invasion Conclusion High percent (40%) PNI positivity in patients with gastric carcinoma who underwent curative resection. Incidence of PNI that was higher in patients having larger tumor (≥5 cm), poorly diff adenocarcinoma, mural invasion, lymph node involvement, advanced stage and lymphatic and venous blood vessels. , however, P value is not significant due to small sample volume compared with the other studies and worldwide gastric cancer database, which was the main limitation of this study.
