Abstract:

Introduction: Amoebiasis is endemic infection in developing countries mostly in the Indian subcontinent, Southern and Western Africa and South America. Intestinal amoebiasis the most common type of amoebiasis infection has a wide spectrum of presentation, ranging from asymptomatic cyst carriage to fulminant colitis. Fulminant necrotizing amoebic colitis is uncommon and only a few cases have been reported in the literature. In fact diagnosis of amoebic fulminant colitis is rarely made preoperatively and it depends mainly on histopathologic study after operation. Case presentation: In this case, patient presented with severe generalized abdominal pain following 1week intermittent diarrhea, abdominal pain and then there was added constipation, vomiting and acute abdominal pain symptoms and sign. He was taken for laparotomy. Total colectomy, proximal ileostomy and exteriorization of distal end of sigmoid as mucous fistula was done. Postoperative state was marked by septicemia and multi-organ failure. We are lost our patient in 1 postop day. Conclusion: For acute amebic colitis, once suspected, early diagnosis and antiamoebic treatment is require because prognosis is poor in developed fulminant colitis. Early diagnosis and surgical treatment significantly decrease mortality rate but conservative surgery has no place in the management of acute FAC, and primary total resection is the choice of treatment.
