Mullerian duct anomalies are those structural anomalies caused by errors in müllerian-duct development during embryonic morphogenesis. Factors that precipitate include genetics, and maternal exposure to teratogens.Mullerian duct anomaly due to some errors in mullerian–dut development during embryonic morphogenesis could be concomitant with urinary anomalies. We report a woman with congenital urogenital anomaly including cervico-vaginal atresia, ureteral sinus and unilateral band shape uterus. A 40-year-old woman with primary amenorrhea and normal secondary sexual characteristics was admitted to a teaching hospital with pelvic pain from last month. She had a surgery on the bladder and urethra in childhood. In physical examination, the urethra was dilated and vagina was absent. Sonography showed an ovarian cystic mass with 86*73 millimeters in diameters with solid component and without flow in Doppler Ultrasound in the right ovary. Left kidney was atrophic and ectopic in pelvic and right kidney was hypertrophic. In MRI, a T2-weited image showed an ovarian cystic mass without enhancing. In laboratory test, tumor markers were normal. On laparoscopy, there were sever adhesion between bladder and omentum due to last surgery. After adhisionolysis we found unilateral band shape uterus in the right side of pelvic, a big cyst in the right ovary and normal ovary and fallopian tube in the left side. The ovary was removed because the imaging suspect the malignancy. The patient had an uncomplicated recovery and was discharged two days after surgery. Pathology confirmed the diagnosis of borderline serous tumor with presence of a mucinous cystadenoma. Finally, we planned the vaginoplasty for her sexual function and ART with gestational surrogate for childbearing.
