Background: Choroid is the most common intraocular site for metastasis because of its abundant vascular supply. Overall, the reported prevalence of uveal metastasis is 2-9%, and breast cancer is the most common cause. This case report describe a 48 year old lady with breast cancer and unilateral choroidal metastasis after 9 years. Case presentation: We report a case of late onset choroidal metastasis from estrogen, progesteron receptor (ER, PR) negative and HER2 positive breast cancer. The patient underwent chemoradiation and herceptin after surgical treatment to remove breast carcinoma. After 9 years she developed decreased visual acuity. Ophthalmologic examination and ultrasound of eye revealed involvement of left side choroid. Brain MRI, chest and abdominal C.T. scan were within normal limits. Radionuclide bone scan showed involvement of right side of pelvis. Now the patient is on external irradiation to right side of pelvis and left orbit. Our plan is to continue treatment with chemotherapy, anti HER2 treatment and bisphosphonate. Conclusion: Here we discuss this case and review the relevant literature. This case mentions the importance of long term follow up examination of breast cancer due to the chance of late onsent recurrence.
