Abstract:

Introduction: Carcinosarcoma of breast (Metaplastic carcinoma) is a rare malignancy of breast with aggressive clinical course. This biphasic neoplasm includes malignant epithelial and mesenchymal elements which can manifest with different histopathologic features. Here we present a case with chondrosarcoma component.Case: A 29 years old female with right breast upper outer quadrant mass from 10 months before admission underwent excisional biopsy. She had noticed the mass two months after her second childbirth. Imaging evaluations were in favor of benign conditions. In macroscopic evaluation a well-defined mass measuring 9 x7.5 x4 cm was seen. Cut sections appeared a creamy-gray colored mass with a cystic area measuring 4 cm in greatest dimension containing gray-brown gelatinous material. Microscopic evaluation of Hematoxylin and Eosin stained slides revealed high-grade carcinoma, cartilaginous matrix production, focal chondrosarcoma, high-grade sarcoma and In situ ductal carcinoma. Frequent mitoses with some atypical mitotic figures, vascular invasion and necrotic areas were also seen. Conclusion: Carcinosarcoma of breast is a rare malignancy with less favorable prognosis compared to conventional breast cancer. It is recommended to take samples from breast masses for determining the exact diagnosis, especially in young females.
