Testicular Tuberculosis Masquerading as A Tumor: A Case Report

Abstract:
Introduction: Testicular tuberculosis is a rare presentation of tuberculosis that presents as a painless swelling of the scrotum. It can be associated with previous tuberculosis infection. Its incidence is on the rise due to human immunodeficiency virus infection. Case Presentation: Here, we present a twenty-four-year-old man with enlargement of his right testicle with no history of fever, malaise, sweating, or cough. The suspicion was a testicular tumor. The patient underwent orchiectomy and pathologic examination was performed. There was a well-circumscribed, soft mass with necrosis. Microscopic examination showed well-formed epithelioid granuloma with acidophil necrosis. The ziehl- Neelsen stain for acid-fast bacilli was positive. Therefore, the diagnosis of testicular tuberculosis was rendered. Conclusion: Tuberculosis should be one of the differential diagnoses in any patient presenting with testicular swelling, especially in developing countries or in the context of immunodeficiency.
