Gliosarcoma with Glandular and Chondrosarcomatous Differentiation: A Case Report

Abstract:

A heterogenous group of CNS tumors are characterized by mixed neuroepithelial and mesenchymal features. Glial tumors manifesting this phenomenon are designated as gliosarcoma. These tumors are usually mistaken for cerebral metastases or meningioma at operation. Histologic study of them reveal an admixture of gliomatous and sarcomatous tissues which lend to a biphasic pattern. Mesenchymal component can present different forms as, fibrosarcoma, undifferentiated pleomorphic sarcoma, chondro-osteogenic and myogenic differentiation, angiosarcomatous, and liposarcomatous types as well. Squamous differentiation, adenoid formations and glandular structures may also be displayed.Herein, we report a rare case admitted at emergency room with decreased level of consciousness in favor of methadon poisoning. Clinical work up show a temporoparietal mass on radiologic investigation. Histopathologic evaluation of brain mass reveal gliosarcoma with glandular differentiation and mesenchymal component which show chondrosarcomatous presentation. Immunohistochemistry study confirmed histology by positive EMA, GFAP,S100 and vimentin expression in different components.
