Calcitonin as an Adjuvant Therapy after Surgical Excision of Central Giant Cell Granuloma of Mandible: A Clinical Case Report

Abstract:

Background and objective: WHO define giant cell granuloma as an intraosseous lesion consisting of cellular fibrous tissue. This benign osteolytic lesion is more common in the anterior mandible in women under thirty. Unilocular or multilocular radiolucency with well-defined borders are its radiographic features. Management of CGCG differs from a simple curettage to complex surgical enucleation. Interferon therapy, administration of calcitonin or steroids are non-surgical treatment protocols for CGCG. Patient report: A 33-year-old woman in Isfahan has the chief complaint of localized intraoral increasing swelling with paresthesia in the left side of the mandible for 3 month. Mild facial asymmetry, bony-hard, reddish, non-tender mass was observed. Panoramic radiograph showed a well-defined intraosseous radiolucent mass with radio opaque borders in anterior mandible leading to root resorption. Under general anaesthesia after excisional biopsy 1 ml calcitonin was injected in the site. The patient received calcitonin subcutaneously for one month after the surgery. Conclusion: A conservative treatment consisting of excision and calcitonin therapy prevent consequences of radical resection. After 6 months follow up no signs of recurrence was observed. This method could be effective substitute for the resection in aggressive CGCG cases to prevent morbidity and loss of function.
