Introduction: MEDA-7 is a novel adipocytokine that its gene expression increases in adipose tissue of obese animals and human. No study is available that identify the changes of Meda-7 gene expression in adipose tissue of animals or human with PCOS and that compare the effects of pioglitazone and metformin on its gene expression in experimental model of PCOS..Materials & Methods: Thirty rats were divided into six experimental groups with five rats in each group: 1) healthy control, 2) PCOS, 3) metformin treated 4) pioglitazon treated, 5) PCOS and metformin treated and 6) PCOS and pioglitazon treated. PCOS was induced by a single i.m injection of 4 mg estradiol benzoate for 40 days. Animals received 200 mg/kg/day and 20 mg/kg/day of metformin and pioglitazone by gavage method for 21 days. Body weight and BMI, serum levels of testosterone, progesterone, glucose, insulin as well as HOMA-IR were measured. Analysis of MEDA-7 gene expression in adipose tissue was performed by real-time PCR.Results: The level of adipose MEDA-7 mRNA was increased in PCOS group compared to the control group. Metformin and pioglitazon attenuated MEDA-7 expression in PCOS groups. The levels of glucose, insulin and HOMA-IR were increased in PCOS group compare to the other groups. We found significant positive association between adipose MEDA-7 gene expression and testosterone, progesterone insulin and glucose level. Conclusion: The increasing of MEDA-7 gene expression may have role in the pathophysiology of PCOs. Metformin and pioglitazone may decrease adipose MEDA-7 expression by improvement of insulin sensitivity
