CLINICAL FEATURES AND MANAGEMENT OF NEWBORNS WITH PASSIVE ADDICTION AND WITHDRAWAL SYMPTOMS: A NARRATIVE REVIEW

Abstract:

Background and Aim : More than half of neonates born from addicted mothers to neuroactive drugs, such as barbiturates and heroin, suffering from Passive addiction and withdrawal symptoms. Withdrawal symptoms usually occur in the first days of birth. The aim of this study was to assess the clinical features of passive addiction and withdrawal symptoms, and therapeutic measuresMethods : This narrative review was performed by searching Keywords such as neonatal abstinence syndrome, substance withdrawal syndrome, and Therapeutics in three electronic databases, including PubMed, Scopus, and Magiran, from beginning of 2017 until the end of 2018. Results : The main manifestations of Withdrawal symptoms included nervous irritability, restlessness, sleep and awakening disorders, and shrill cry. Other manifestations were digestive, cardiac and dermatological. The key to treatment is the rapid diagnosis of the disease. The most important therapeutic plans are cardiovascular, respiratory, and renal management, and babies need to be monitored in terms of fever, fluid and electrolyte, sepsis, and metabolic disorders. The most commonly used drugs are chlorpromazine, diazepam, tincture of opium, and phenobarbital. It is also important to pay attention to baby nutrition.Conclusion : Regarding the high probability of developing withdrawal syndrome in neonates born from addicted mothers to neuroactive drugs, these mothers should be instructed to inform their physician of their addiction. On the other hand, due to the long recovery time, the use of experienced medical team in managing the symptoms of withdrawal can increase the likelihood of treatment.
