The effect of botulinum toxin in women with overactive bladder and urge urinary incontinence

Abstract:
Objective:To determine the effect of botulinum toxin type A injection into the detrusor muscle of bladder regardingthe quality of life (QOL) and symptoms of female patients with overactive bladder (OAB) and urge urinaryincontinence.Methods:Seventeen postmenopausal women with OAB and urge urinary incontinence underwent cystoscopy and20 botulinum toxin type A detrusor injections were done (0.5 cc at each injection). Urinary incontinenceepisodes during daytime and nocturnal urination episodes as well as QOL using Incontinence Quality ofLife (IQOL) Questionnaire were evaluated one and six months after the injections.Results: The average number of urinary incontinence episodes during daytime at baseline (before theinjections) was 13.25 which decreased to 5.87 (month 1) and 6.12 (month 6); P = 0.027. The averagenumber of nocturnal urination episodes at baseline was 7.12 which decreased significantly to 3.75 and3.5 in the first and sixth months, respectively (P = 0.007). Baseline mean I-QOL total score was 13.59 whichreached 78.87 (month 1) and 82.12 (month 6); P < 0.001. The only complication reported was urinary tractinfection seen in two patients (11.76%).Conclusion: Botulinum toxin type A detrusor injection had a significant effect on the improvement ofsymptoms of female patients with OAB and urge urinary incontinence as well as improvement of theirQOL. These effects were observed after a month and at the sixth month after treatment, the effects ofthe recovery were persistent.
