Abstract:

Background and Aim : One of the most common cause of maternal mortality in the first trimester ofpregnancy is the ectopic pregnancy. It seems that early diagnosis of ectopic pregnancy (EP) is importantdue to life-threatening consequences in the first trimester of pregnancy. Adenosine deaminase (ADA) is anessential enzyme for the production, maturation and function of lymphoid cells which is produced in alltissues of the body. Total serum ADA levels decrease during normal pregnancy compared to non-pregnantstate. This study aimed to investigate the relationship between ADA and EP in pregnant women referred toShahid Motahari hospital in Urmia. Methods : This case-control study was performed to compare serum levels of ADA in people with andwithout ectopic pregnancy in Shahid Motahari hospital in Urmia at 2018-2019.In this study, 94 pregnantwomen were enrolled, 47 patients as control group with normal pregnancy and 47 patients as case groupwith ectopic pregnancy. The level of βHCG and serum ADA levels were compared in two groups. The datawere analyzed by SPSS20 using descriptive and inferential statistics .P–value less than 0.05 was significant. Results : The mean ADA level in patients with ectopic pregnancy was 12.21 ± 8.17 IU / L and in the controlgroup was 8.44 ± 6.21 IU / L (P = 0.01). The mean βHCG level in women with ectopic pregnancy was3215.60 ± 1400.71 mIU/ml and in women with normal pregnancy was 11926.96 ± 3408.23 mIU/ml(P=0.001). There was a significant difference between the level of βHCG in women with ectopic pregnancyand normal pregnancy and was a significant difference between ADA levels between the two study groups. Conclusion : According to the results of our study, high levels of ADA can be very useful in early diagnosisof ectopic pregnancy, and could be useful as a predictor of risk of ectopic pregnancy. It is also recommended that women who have an ectopic pregnancy have an ADA test done within the first few weeks of pregnancyto determine if they have ADA abnormalities and to take appropriate follow-up and termination ofpregnancy to avoid potentially dangerous complications of ectopic pregnancy.
