Abstract:

Background and Aim : To evaluate the effectiveness of vasopressin injection technique duringlaparoscopic cystectomy on ovarian reserve in patients with unilateral endometriomas.Methods : This study was a randomized prospective clinical trial, in which 136 women aged 18-42 yearswith unilateral endometriomas were randomly divided into two groups with injection of diluted vasopressinfor the case group and no injection for the control group. Blood sampling was also done to measure anti-Mullerian hormone (AMH) and hemoglobin (Hb) before and 6 months after surgery to predict ovarianreserve and blood loss.Results : In the case group, the mean level of AMH (2.67±1.50) after surgery decreased as compared tobefore surgery (3.43±2.46), indicating there is no statistically significant difference in this regard beforeand after the surgery (P= 0.07). In control group, the mean level of AMH (1.61±1.79) after surgery waslower than before surgery (3.49±2.48), suggesting there is a significant difference in this regard before andafter the surgery (P= 0.04). In the case group, the mean volume of blood loss was lower than the controlgroup (P=0.04).Conclusion : Vasopressin injection is an ideal procedure to decrease damages to ovarian
