Abstract:

Introduction: OCD (Obsessive-Compulsive Disorder) is a serious mental health disorder that can have severe and disabling effects on individuals. Many individuals with OCD do not respond well to treatment, leading to treatment-resistant OCD. The aim of this study is to gather and evaluate new approaches for the management of treatment-resistant OCD.Method: In order to find relevant data from ۲۰۱۸ to ۲۰۲۳, searches were conducted in databases such as PubMed, Google scholar, SID and Scopus. The search included key words such as "OCD", "Treatment" and "Drug resistant".Results: This review article provides an overview of recent studies and research findings related to the treatment of obsessive-compulsive disorder (OCD) and treatment-resistant OCD. The article providing documents comparing the efficacy of clomipramine with other commonly used medications for OCD, as well as clinician perspectives on deep brain stimulation (DBS) as a potential intervention for treatment-resistant childhood OCD. We also discusse the use of rituximab, transcranial magnetic stimulation (TMS), electroconvulsive therapy (ECT), and other treatment that showed efficacy for SRI-resistant OCD. our findings focus on the effectiveness of various interventions, such as DBS, TMS, and cognitive-behavioral therapy, in improving symptoms and overall functioning in patients with treatment-resistant OCD. We additionally discusse the use of novel pharmacological approaches such EPD۱۵۰۴, mirtazapine augmentation, lamotrigine and aripiprazole augmentation, and lurasidone as an adjunctive therapy, as a potential options for managing treatment-resistant OCD. Finally, This article also looks at the effectiveness of rTMS in patients who have or have not developed a resistance to SSRIs, also the impact that sleep disruptions and rTMS play in treating OCD.Conclusion: These review provides a better understanding of the new available treatment options and management strategies for patients with OCD and treatment-resistant OCD.
