Abstract:

Background: The role of the breast clinics is to diagnose breast cancer andreassure patients with benign disorders. One stop clinics - same day reporting -further reduce anxiety caused by the delay in the results. The first one stop clinicwas introduced inSQUH–Omanin September ۲۰۱۱.Methods: This retrospective analysis of ۳۹۵ consecutive cases presented toBreast One Stop Clinic at SQUH was done between September ۲۰۱۱ andDecember ۲۰۱۳. All patients underwent triple assessment conducted byconsultants. Mammography was performed for patients above ۳۵ years with nocontraindications. Ultrasound was done for all patients with palpable lesions. Thereports were assigned a score (BIRADS ۰-۶). Fine Needle Aspiration Cytology(FNAC) was done for all patients with palpable abnormality or nipple discharge.An immediate report was given using the standard reporting categories:acellular/inadequate, benign, atypical, suspicious, and malignant (C۱-C۵).Results: Out of ۳۹۵ patients, ۲۱۰ were found to have palpable lesions with amean age of ۳۶ years (۱۲-۸۴ years). All patients were female. The FNACsensitivity and specificity was ۹۵.۷% and ۹۲.۲%, respectively. Out of ۲۱۰ patients,۱۵.۳% were diagnosed with breast cancer on FNAC who were subsequently stagedand discussed in the Breast Multidisciplinary Team Meeting (MDT), and ۸۴.۷%were diagnosed to have benign breast disorders who were reassured and advised tobe followed up after ۶ months.Conclusions: Immediate relief and reassurance to the majority of patients withbenign disease obviate the need for review appointments.Areliable diagnosis canbe obtained with triple assessment which can be used to avoid unnecessary corebiopsy of benign lesions, if correctly done.
