Abstract:

Introduction: Early postoperative pain is a common complaint after laparoscopic cholecystectomy andpersistent pain after operation is a dominant complaint and primary cause for prolongation of hospitalizationafter surgery. Major part of pain management after surgery is related to opioids. Trends to non-opioids as apart of multimodal analgesic approach to reduce pain of patients following various surgical procedures isexisted. In this study, we evaluated the efficacy of preoperative single dose pregabalin versus coadministrationof pregabalin and celexib to reduce the postoperative pain in patients referred to Kashanihospital, Shahrekord, who underwent laparoscopic cholecystectomy under generalized anesthesia.Methods: This was a double blind placebo-controlled clinical trial in which patients underwent electivelaparoscopic cholecystectomy referred to Kashani hospital, Shahrekord and met the inclusion criteria, wereenrolled. Patients were randomly allocated to control (C; received placebo), group B (only ۱۵۰ mg pregabalin)and group A (received ۱۵۰ mg pregabalin and ۲۰۰ mg celexib together), after obtaining written informedconsent. For each patients, checklist were completed including demographic data, blood glucose, nausea andvomiting, need to use analgesic, vital sign, visual analogue score (VAS) of pain, numerical analogue score(NAS) for pain in different time (۰, ۲, ۸ hours after surgery). Finally, outcomes were analyzed.Results: Totally, ۶۰ patients were included in the study and ۲۰ patients in each groups. The mean age ofparticipants was ۵۰.۹۰±۹.۰۶ years old. Most of them were female (۴۱, ۶۸.۳%). There was significantdifference between group A and B in VAS score, ۲ hours after surgery (P=۰.۰۱۶) but not in other rimes(P>۰.۰۵). The mean of VAS and NAS in ۲ and ۸ hours after surgery was significantly lower in group A and Brather than group C (P<۰.۰۵). The mean time of hospitalization in group C (۳.۹۵±۰.۸۸ days) were significantlylonger than group B (۱.۹۵±۱.۲۷ days) and A (۱.۷۰±۰.۸۴ days) (P=۰.۰۰۱).Conclusion: Overall, our findings showed that oral use of pregabalin only and pregabalin and celexibcombination despite of pain improvement rather than control group, had no difference between them clinicallyand statistically, and both of them can significantly decrease postoperative pain.
