The Effect of Communication using the Peplau's Theory on Satisfaction with Nursing Care in the Elderly Hospitalised in the Cardiac Intensive Care Units

Abstract:

Introduction: Increasing satisfaction with nursing care and education to increase understanding of the type of disease, treatment process, drugs used, and follow-up can improve quality of life, hope for the future, and independence in self-care. As a result, the current study aimed to determine the effect of communication using Peplau's theory on nursing care satisfaction in the elderly hospitalised in cardiac intensive care units in hospitals in Guilan Province’s east.Material and Methods: This quasi-experimental study was conducted by available sampling methods on ۷۸ hospitalised older adults (۳۹ interventions and ۳۹ controls) in the cardiac intensive care units of Guilan hospitals in ۲۰۲۱. In the intervention group, Peplau’s communication theory (four stages of orientation, identification, exploitation, and resolution) was implemented, and the control group received routine care. At the time of discharge, a patient satisfaction questionnaire about the quality of nursing care was completed for both groups. Data were analysed using SPSS version ۲۰ and a descriptive statistics and independent t-test.Results: The mean age in the intervention group was ۷۰.۸±۶.۳ and in the control group was ۷۳.۸±۸.۸, and in general it was ۷۲.۳۰±۷.۶. Half of the participants were men, and the other half were women. The mean score of satisfaction with nursing care in the intervention group (۹۳.۰ ± ۵.۴) was significantly higher than that in the control group (۷۵.۷ ± ۶.۸) (t=۱۱.۵۴, p<۰.۰۰۱, d=۲.۸۴). In other words, the mean score of satisfaction with nursing care in the intervention group was ۱۷.۴ units (۹۵% confidence interval: ۲۰.۴ - ۱۴.۴) higher than in the control group.Conclusion: The use of Peplau's communication theory has been effective in increasing the satisfaction of nursing care among the elderly hospitalised in cardiac intensive care units. Using this communication method in the care of these patients can lead to an improvement in the level of satisfaction and quality of care, increase functional independence, and reduce the need for care.
