Risk Assessment of Runway Excursion Using Bowtie Technique: A Case Study in One of the Iran's International Airports

Abstract:
One of the most significant problems that various industries face today is work-related accidents, which come with high costs. According to the International Labor Organization (ILO) report, there are over ۳۵۰,۰۰۰ fatal occupational accidents, approximately ۳۱۳ million non-fatal occupational accidents, and nearly ۲ million work-related illnesses estimated. According to the information from the aviation safety network in Iran, until December ۲۰۱۹, ۲۱۵۲ peoples lost their lives in ۶۲ aviation accidents. In this study, to reduce risks and accidents, the Papillon method, which is a combination of two methods of fault tree analysis and event tree analysis, was used to identify various causes of accidents and determine their probability of occurrence. The consequences of aircraft departure from the runway include damage to the airport environment and its equipment, damage to the aircraft, or harm to individuals. Each of these consequences was examined and analyzed by drawing the Bowtie diagram of the accident path from the beginning of the causes to its consequences. In total, ten causes were identified, and a solution was proposed for each one. As a result, all information related to accident occurrences and factors for their elimination was examined, and based on the risk assessment matrix, the risk ranking for their occurrence was obtained.
