Performance of Wrist-Worn Pulse Oximeter for the Screening of Obstructive Sleep Apnea

Abstract:
Introduction:A sleep apnea monitor (BM۲۰۰۰A) is a wrist-worn device that measures oxygen saturation and pulse rate during sleep. This study aimed to evaluate the efficacy of the watch-like BM۲۰۰۰A for screening obstructive sleep apnea (OSA).BM۲۰۰۰A; Home sleep apnea testing; Obstructive sleep apnea; Polysomnography; Wrist pulse oximeter Materials and Methods:۱۰۲ patients complaining of sleep breathing disorders were included; ۸۱% were men and ۱۹% were women. All participants underwent overnight simultaneous polysomnography (PSG) and BM۲۰۰۰A sleep monitoring. The number of apneas and hypopneas, apnea-hypopnea index (AHI), percentage of time spent with oxygen saturation under ۹۰%, average oxygen saturation, lowest oxygen saturation, and duration of sleep were computed by the BM۲۰۰۰A and PSG. Then, these parameters were compared to validate the BM۲۰۰۰A.Results:All parameters, measured with BM۲۰۰۰A, had a good correlation (r ≥ ۰.۶, p < ۰.۰۰۰۱) with PSG-derived indexes, except for sleep time (r = ۰.۱۹, p = ۰.۰۶۱) and hypopnea index (r = ۰.۴, p < ۰.۰۰۰۱). AHI had the strongest correlation (r = ۰.۸۷, p < ۰.۰۰۰۱). The mean difference between AHI values calculated with PSG and wrist-worn pulse oximeter (WPO) was -۱۷.۶۶ events/h (۹۵% CI: -۵۰.۳۹ to ۱۵.۰۶). In AHI ≥ ۵, BM۲۰۰۰A had ۹۰.۷% sensitivity, ۱۰۰% specificity, ۹۱.۲% accuracy, and ۰.۹۹۴ area under the curve. Using AHI ≥ ۵, ≥ ۱۵, and ≥۳۰ as the screening criteria, optimal WPO-AHI cutoffs to improve the screening accuracy were ۳.۱۰, ۸.۹۲, and ۱۳.۰۵.Conclusions:BM۲۰۰۰A-derived results properly correlate with PSG and can provide OSA screening with good sensitivity and specificity.
