Surgical Techniques for Upper Gastrointestinal (GI) Cancers abstract
This article explores the evolving surgical techniques for treating upper gastrointestinal (GI) cancers, focusing on three primary approaches: traditional open surgery, minimally invasive surgery (MIS), and specialized techniques such as cytoreductive surgery combined with hyperthermic intraperitoneal chemotherapy (HIPEC). Open surgery remains the cornerstone for advanced upper GI malignancies, providing direct access for comprehensive tumor resection and complex reconstructions. MIS, including laparoscopic and robotic-assisted methods, offers reduced postoperative pain, shorter recovery times, and improved cosmetic outcomes through small incisions and precise instrumentation. Robotic-assisted surgery enhances the capabilities of MIS with superior visualization and control, making it ideal for intricate procedures. Meanwhile, endoscopic surgery enables the resection of early-stage cancers through natural orifices, minimizing invasiveness and promoting faster recovery. Cytoreductive surgery with HIPEC is specialized for advanced cases involving peritoneal metastasis, combining extensive tumor removal with localized chemotherapy to target residual disease. Each technique is evaluated for its efficacy, patient outcomes, and appropriate application, reflecting advancements in surgical oncology that improve survival and quality of life for patients with upper GI cancers.
