Assessment of breast mass imaging modalities (Mammography , Sonography , MRI) and pathological reports (FNA, Core needle biopsy, Open biopsy ) compared to permanent biopsy, Shiraz abstract

Background: Breast cancer is the second prevalent cancer in world and the most prevalent cancer in women. Imaging screening helps detection of cancer in early stages and pathological modalities use to confirm diagnosis. Early diagnosis of breast cancer decreases mortality and irritation from disease. Also it’s important for doctor to know how much can rely on breast imaging (MG, US, MRI) and pathological modalities (FNA, CNB, Open biopsy). In this study, we assess accuracy of breast mass imaging modalities (MG, US, MRI) compare to permanent pathology and accuracy of breast mass pathological modalities (FNA, CNB, Open biopsy) compare to permanent pathology. Patients and Methods: Retrospectively; we studied pre operation breast imaging (MG, US, MRI) and pathological reports (FNA, CNB, Open biopsy) and permanent pathology of six thousand and seven hundred sixty five (6765) cases with breast mass who referred to Breast Diseases Research Center affiliated with Shiraz University of Medical Sciences since 1999 to 2017. The study was based on the type of mass due to malignancy, by considering this issue we had 2564, 3428, 37, 3922, 1058, and1234 available findings of MG, US, MRI, FNA, CNB, and Open biopsy, respectively. We calculated sensitivity, specificity, positive predictive value(PPV), negative predictive value(NPV) and accuracy of imaging and pathological modalities of breast mass compare to permanent pathology. Statistical analysis was performed by SPSS. Results: Our study showed that sensitivity is 88.43%, 93.1%, 72.72%, 90.73%, 96%, and 97.82% in MG, US, MRI, FNA, CNB, and Open biopsy, respectively. Specificity is 66.67%, 66.67%, 50%, 50%, 25%, and 0% in MG, US, MRI, FNA, CNB, and Open biopsy, respectively. PPV is 99.87%, 99.9%, 88.89%, 99.96%, 99.7%, and 99.91% in MG, US, MRI, FNA, CNB, and Open biopsy, respectively. NPV is 1.92%, 2.59%, 25%, 0.36%, 2.43%, and 0% in MG, US, MRI, FNA, CNB, and Open biopsy, respectively. Accuracy is 88.36%, 93.03%, 69%, 90.70%, 95.72%, and 97.74% in MG, US, MRI, FNA, CNB, and Open biopsy, respectively. Conclusion: Among imaging modalities; sonography has the most sensitivity and accuracy. Specificity of MG and US is the same and more than MRI. Among pathological modalities; Open biopsy has the most sensitivity and accuracy.
