Scalp Actinomycosis abstract

مقدمه: Actinomycosis is chronic granulomatous or suppurative infection caused by anaerobic or microaerophilic bacteria named genus Actinomyces. The most common site of infection is cervico-facial. It is characterized by suppuration, abscess formation, tissue fibrosis, draining sinuses, sulfur granules and rarely as soft tissue mass mimicking tumor. This is the rationale for publishing this case report to make clinicians aware that tumoriform actinomycosis of the scalp is rare entity that miss diagnosed as neoplasm. معرفی بیمار: 35 year old man with history of blunt head trauma since he was adolescent. Three years later the patient presented with mass and swelling on right temporal area of his head. He had sinus on head with yellowish discharge and pain in winter that it progressed and presented with soft tissue tumor like lesion since months ago. Ct scan of head showed soft tissue lesion without bone involvement. Treatment with Penicillin was done. Excisional biopsy was carried out and histopathology report showed dense inflammatory cells with colonies of gram positive filamentous bacteria consistent with actinomycosis. In follow up the patient had not any problem. بحث نتیجه گیری: Tumoral presentation of Scalp Actinomycosis as pseudocarcinomatous or sarcomatous masses is very rare. Lesions not resolving with routine antibiotics therapy should be suspected clinically as actinomycosis.Diagnosis is based on biopsy and laboratory diagnostic methods such as histopathology study, direct examination and culture.Treatment is done by oral or IV antibiotics.Surgery maybe needed.
