Hodgkin lymphoma in 13 years old case with rare manifestation of HSP-like purpura

مقدمه: Lymphoma is blood cancer that develop from lymphocytes. It’s the most common form of hematological malignancy in developed world. Lymphoma represents about 5% of all cancers and about 55% of hematologic cancers. Hodgkin lymphoma accounts about 15% of lymphomas and it differs from other lymphomas in its diagnostic manifestations, prognosis and treatment. Pathologically Reed-Strenberg cell is hallmark for Hodgkin lymphoma.• معرفی بیمار: 13-year-old male with upper respiratory infection signs and symptoms that didn’t respond to Amoxicillin and ‎Aminoglycoside was admitted to our hospital. Five days later with manifestations of skin Petechiae and purpura in lower limb, arthralgia and high CRP and ESR it was diagnosed as Henoch-Schonlein purpura medical treatment cured HSP but ESR remained elevated. Because of high ESR and abdominal pain patient was admitted at department of Gastroenterology but no sign of IBD or Crohn disease were seen and patient was referred to oncology department. Chest X-ray revealed para-hilar and para-tracheal masses. CT with contrast showed lymphadenopathy in anterior right thorax area. Finally pathological evaluation and flowcytometric immunophenotyping analysis of masses revealed CD30+, CD15+ and CD45- lymphoma that was diagnosed as classic Hodgkin lymphoma. Now patient is on chemotherapy.• بحث نتیجه گیری:Common symptoms in Hodgkin lymphoma are LAP (lymphadenopathy) in neck, intermittent fevers, drenching night sweats, tiredness, etc. HSP-like purpura is not usually diagnostic for Hodgkin lymphoma especially in children. But in this novel case the patient only showed manifestations of HSP which maked the diagnosis difficult.
