Quercus Brantii Lindl. vaginal cream versus Placebo on Bacterial Vaginosis: a randomized clinical trial

Abstract:
Background: The acidic milieu of the vagina is maintained by lactobacilli, which account for 95% of the bacteria found in the vaginal flora and inhibit the growths of other vaginal commensals in normal conditions. Lactobacillus species that produce hydrogen peroxide usually settle in normal vaginal flora, while Mobiluncus species, anaerobic gram-negative rods, Gardnerella vaginalis, and Mycoplasma hominis take their places in the case of Bacterial Vaginosis (BV). BV frequently leads to a vaginal discharge in 20-50% of the involved women while half of them remain asymptomatic. Current study aimed to compare the effects of Quercus brantii Lindl. vaginal cream in combination with metronidazole tablet on bacterial vaginosis Methods: Totally, 176 eligible women with BV were enrolled. Diagnosis was based on having 3 or more signs described by Amsel’s criteria and a Nugent score of ≥7. Symptoms were evaluated before and after treatments. Participants were divided into two experimental groups based on permuted-block randomization. Group A (n=89) received 500 mg of metronidazole tablet each 12 hours besides a vaginal placebo cream. Group B (n=87) received 500 mg of the same tabletwith a vaginal Quercus brantii cream. Seven to 10 days after the treatments, Amsel’s criteria Nugent scores, and the clinical symptoms were assessed again. The data were analyzed via chisquare, independent, and paired t-tests using SPSS software. P<0.05 was considered to indicate significant differences between the treatments Results: According to the data obtained from our study, the most common reason for the participants (168 women) looking for a treatment was vaginal discharge (34.9 %). Malodordischarges (15.7%) and itching (12.4%) were other reasons. The mean (SD) ages of the participants in groups A and B were 40.70±9.59 and 42.39±8.68, respectively. After thetreatments, significant differences were observed between the two groups in terms of malodor discharge and Nugent score. Based on the total cure, 50.0% of the patients in group A and 73.8% of those in group B were found to have been treated when assessed by both Nugent score and Amsel’s criteria. Conclusion: The findings suggested the better results of metronidazole+Quercus brantii vaginal cream compared to those of metronidazole+placebo
