Assesment the effect of vaginal cyclogest on the ressistance of fetal-uterine vessels in preterm labor

Abstract:
Background and Aim : Preterm labor is one of the most important causes of prenatal mortality andmorbidity among pregnant women. Considering the importance of progesterone in the continuation ofpregnancy, the probable role of its vasodilatation effect on fetal-uterine vessels, and the limited studies onthis subject, the aim of this study is set to evaluate the effects of vaginal cyclogest on the resistance of fetaluterinevessels in preterm labor.Methods : To achieve the goal of the present study, a randomized clinical trial was performed on 90 womenwith gestational age of 24 to 33 weeks, who were at the risk of preterm labor in Qazvin Kowar Hospital in2016-17. Patients were randomly divided into the case and control groups. The case group was treated witha single dose of 400 mg of vaginal cycloges. Both groups were evaluated with Doppler ultrasonographybefore and 24 hours after the treatment, and the values of Resistance Index (RI) and Pulsatility Index (PI)of the uterine artery, umbilical artery, and fetal middle cerebral artery were recorded.Results : The results of the study showed that although the treatment with vaginal cyclogest reduced theincidence of preterm labor, the change was not statistically significant (P> 0.5). It was observed that thechanges in RI index of uterine artery after treatment were statistically significant (P=0.009). In addition, RIand PI indices of fetal middle cerebral artery showed a significant difference after treatment (P<0.0001 andP=0.002, respectively). However, the changes in the RI and PI indices of umbilical artery and the changesin the PI index of the uterine artery after treatment were not statistically significant (P> 0.05).Conclusion : According to the results of this work, as well as other studies, progesterone, with itsvasodilator properties, reduces the parameters of arterial resistance of fetal-uterine vessels, and thereforemay be useful in preventing preterm labor.
