Abstract:

The growth of high-risk industries such as oil, gas, and petrochemicals industries along with the occurrence of catastrophic disasters in recent years has led researchers to seek to establish a mechanism to prevent or reduce the consequences of accidents. In this study, the main and basic concepts for creating instructions and performing incident investigation are first explained. For this purpose, topics such as the importance and objectives of incident investigation, the definition of terms used in the analysis, key concepts related to the analysis, such as risk management and control system, release of chemicals or energy, etc., as well as the steps of a successful investigation are described. Then, theories of incident investigation and analysis of commonly used tools in this area are described. Afterward, up to ۲۰ methods of incident investigation are reviewed, and their strengths and weaknesses are compared using the features and requirements stated in different literature. Among all the methods, BSCAT, Tripod-Beta, and Kelvin TOP-SET methodologies were reckoned as the best tools for investigating incidents.
