Abstract:

Background: This study was carried out to determine the effects ofdexmedetomidine on clinical outcomes and renal function after coronary arterybypass grafting (CABG) to address the increased rate of CABG? and theimportance of modification in stress responses and control of adverse effects onrenal function and with the aim of cost reduction.Materials and Methods: This double-blind randomized clinical trial wascarried out with the participation of ۱۲۹ subjects under elective CABG in RajaeiHeart Center, Tehran, Iran in ۲۰۱۷. Patients were randomly assigned to fourgroups with simple randomization including one placebo (normal salineinfusion) group and three dexmedetomidine groups; ۰.۵ μg/kg/h on CPB, ۰.۵μg/kg/h in ۲۴ hours, and ۰.۷۵ μg/kg/h in ۲۴ hours. The blood pressure, bloodurea nitrogen (BUN), serum creatinine, transfused blood volume, urine volume,and hemofiltration on pump and lactate were assessed at different time intervals.Results: Results showed more stability especially in central venous pressure(CVP) (p=۰.۰۰۱) and systolic blood pressure (p=۰.۰۰۶) in the groupsreceiving dexmedetomidine ۰.۷۵ μg/kg/h per ۲۴ hours. But diastolic bloodpressure and heart rate were the same across the groups (p>۰.۰۵). All othervariables including hepatic and renal function tests were the same across thegroups (P>۰.۰۵).Conclusion: Overall, according to the obtained results in the current study, itmay be concluded that dexmedetomidine would have some promising effectson hemodynamic stability but there are no obvious renoprotective effects forthis medication.
