Abstract:

Introduction:Hypertension is one of the major and modifiable risk factors for cardiovascular diseases.To prevent or delay the development of its complications, continuous and long-term care is needed.Methods:We conducted a systematic review, using the key terms of hypertensive patient’s care, nurse, hypertension management in PubMed,Google Scholar,SID,and Scopus(studies published from ۲۰۱۵-۲۲ years,both in Eng and Per)Results:Analyzing the studies showed that to organize and target nursing care for hypertensive patients, first a team of trained nurses should be formed.They perform relevant interventions for hypertensive patients such as nursing consultations, home visits, telephone follow-up, and appropriate referrals to doctors,nutritionists,psychologists.In every period, nurses examine blood pressure,drug adherence,dietary,physical examination....After assessing the patient’s knowledge and behavior, they educate self-management, such as salt intake control, regular physical activities and accurate home BP monitoring. If the patient reported an increased BP in home BP monitoring, the nurse should assess their adherence, any current illnesses or living circumstances that may affect their BP. If the patient had symptoms like headache, visual disorders,he/she refer to the general practitioner to further health check.Conclusion:Nurse-based hypertension management model is a cost-effective method that can help increasing the number of patients receiving ongoing high-quality care
