Abstract:

Since the human error in cementing operation in Petroleum Wells results in catastrophic consequences, identifying and evaluating human errors, which are the main cause of most incidents, is necessary in the operation. This paper investigated human errors in cementing services in petroleum wells using CREAM Technique. The basic and extended CREAM method was applied in this research. In doing so, the main tasks and sub-tasks were identified in cementing operations. Then, control modes, common performance condition, cognitive failure probability were determined. The results showed that %62.5 of the control modes were tactical, %25 of the strategic modes and %12.5 of the opportunistic mode. The time of day, Number of simultaneous goals and sufficiency of training and experience are considered as factors reducing performance. Conclusively, in order to reduce the probability of human error in cementing operations, it is essential to decrease the performance reduction factors and turn opportunistic and tactical control mode into strategic control one.
