BK nephropathy in kidney transplant recipient

مقدمه: There are many reasons for renal transplant dysfunction. Viral infetion with CMV or BK virus is potential risk factor for renal transplant dysfunction• معرفی بیمار: patient is 70 year old female who had come with chief complaint of fever, diarrhea and rise of serum creatinine to mg/dl. She is known case of kidney transplant from cadaveric donor since years ago. she was admitted and she underwent labratory tests and imaging. she developed leukopenia and her serum creatinine increased up to more than mg/dl. she received broad spectrum antibiotics and ganciclovir. Her immunosuppresive drugs were modified. Diarrhea and leukopenia improved by treatment but kidney dysfunction remained. Extensive work up incluling CT scan, endoscopy, colonoscopy ,echocardiography, BM BX and PCR for CMV DNA were done. There was no important finding. She underwent renal transplant biopsy. There was no evidence of rejection or glomerular injury, evidence of cytopathic changes was found. Her assessment for BK virus DNA was positive. She was discharged with decreased dose of immunosupressives. بحث نتیجه گیری:BK virus infection in immunosupressed patients could affect the function and survival of kidney transplants .No evidence exists of specific effective treatment for this infection. Only reduction in immunosuppression treatment can minimise virulence
