URINARY RETENTION CAUSED BY ABDOMINAL AORTIC ANEURYSM abstract

IntroductionAcute urinary retention (AUR) is the inability to voluntarily pass urine. It is the most common urologic emergency. In men, AUR is most often secondary to benign prostatic hyperplasia (BPH). (1)• Case Report 91 year-old-man complaining of acute urinary retention with BPH history was admitted to the emergency room of Rajaei hospital. Vital signs were: BP: 123/76, PR: 93, RR: 18, Spo2:93%. Clinical examinations revealed mass in hypogastric region which seemed as bladder bulging, so bladder catheterization was done emergently which 2.5 of urine was discharged intermittently. Despite the catheterization and high urine volume discharge, the hypogastric mass remained yet. Vital signs changed to BP: 83/50, PR: 80. These evidences lead to renal, urinary tract and bladder ultrasonography by radiologist in which left renal moderate hydronephrosis was reported. In addition, the positive note of laboratory data was Cr: 1.8, Hb: 10.3 (M.C.V:87.1).Revealing hypogastric mass’s pulsatile attitude an ultrasonography was done by emergency physician to examine vascular condition which resulted in abdominal aortic aneurysm (AAA) measuring 10 cm accompanying with intramural thrombosis. After taking primary measurements, by counseling the vascular surgeon, CT angiography of thorax, abdomen and pelvis was done which confirmed AAA and he was administered to surgery. Unfortunately, he faced cardiac attack in which despite the CPR efforts, he expired.• DiscussionRuptured abdominal aortic aneurysm classically presents with abdominal pain radiating to the back, pulsatile abdominal mass and circulatory collapse. (2) In some rare cases like this it is accompanied with urinary retention. So it is better to think to abdominal aortic aneurysm in each ED- referring patient complaining of acute urinary retention and then take the necessary measurements
